Job Details:
Responsible for providing skilled nursing services to patients in their home under the direction of a physician. Must assess, plan, implement, document, evaluate and teach patient's needs and care.
Licensure/Certification Requirements:
Requires current state nursing license and a valid driver's license. CPR certification is required.
Education Requirements:
A graduate of an approved school of professional nursing. One year experience in a related field is required. One year (minimum) of Home Care experience required.
Scheduling Expectations:
Employee will primarily be scheduled for the day shift. However, in order to meet the 24 hour per day, 7 day per week operation at SAHC, employee will be required to participate in the 24 hour "on-call" schedule. Employee is also required to participate in weekend and holiday rotation. Employee must independently schedule patient visits in timely and efficient manner; checking in with office daily to update schedule.
